Modifier 25
Significant, Separately Identifiable E/M Service
E/M on same day as procedure
Important: Modifier usage depends on documentation and payer policy. Never add a modifier without supporting documentation. Always verify payer-specific rules, as modifier policies vary. The information on this page is for educational purposes only and does not guarantee payment. When in doubt, consult the official CPT guidelines, NCCI Policy Manual, and payer-specific policies.
Modifier Description
Modifier 25 indicates that a significant, separately identifiable evaluation and management (E/M) service was performed by the same physician on the same day of the procedure or other service.
Why This Modifier Exists
Modifier 25 was created to prevent payers from automatically bundling an E/M service into a same-day procedure's global package. Before modifier 25 existed, payers routinely denied the E/M as inclusive to the procedure, even when the visit was for a completely separate problem. CMS and AMA recognized that a patient can present with a distinct medical issue AND need a procedure on the same day β modifier 25 makes that scenario separately billable when properly documented.
Plain English Explanation
When you see a patient for a separate medical issue AND also do a minor procedure on the same visit, you add modifier 25 to the E/M code to tell the payer "the office visit wasn't just for the procedure β there was a separate, distinct reason for the visit."
When to Use
Use on an E/M code (99201-99215, 99291-99292) when a significant, separately identifiable E/M service is performed on the same day as a procedure with a global period of 0 or 10 days. The E/M must be above and beyond the usual pre- and post-operative care included with the procedure.
When NOT to Use
Do NOT use modifier 25 if the E/M service resulted in the decision to perform the procedure β use modifier 57 instead. Do NOT use for procedures without global periods unless the E/M is truly separate. Do NOT use on procedure codes β it belongs on the E/M code.
Documentation Requirements
The medical record must clearly document a separate, identifiable E/M service beyond the normal pre- and post-operative care. The documentation must support the level of E/M billed and show the chief complaint, history, exam, and medical decision-making for the separate issue.
Common Denial Reasons
- CO-97 (bundled β payer believes E/M is included in procedure), CO-16 (missing/incomplete information β documentation doesn't support separate E/M), CO-50 (not medically necessary), CO-151 (payment adjusted β same day service).
Common Rejection Reasons
- Claim rejected by clearinghouse for invalid modifier format.
- Rejected for missing place of service.
- Rejected for invalid provider NPI on the E/M line.
- Electronic rejection if the E/M code and procedure code are on the same line instead of separate lines.
Common Mistakes
- Appending to the procedure code instead of the E/M code.
- Using it when the E/M only resulted in the decision to do the procedure (should use 57).
- Not documenting a separate chief complaint.
- Using it routinely on every visit with a procedure.
Example Scenarios
Patient presents for a rash and during the same visit the physician also removes an irritated sebaceous cyst.
The E/M for the rash evaluation gets modifier 25; the cyst removal is billed separately.
Patient presents for a medication check and the physician also performs trigger point injection.
The E/M gets modifier 25.
Payer Considerations
Medicare follows NCCI edits β modifier 25 is allowed when NCCI edits show it as appropriate. Some payers require that the E/M be a different diagnosis from the procedure. UHC and Aetna have specific modifier 25 policies and may request records. BCBS plans vary by state. Check each payer's specific policy.
Telehealth Considerations
Modifier 25 can be used with telehealth E/M codes (e.g., 99213 with modifier 95 and 25). The same documentation requirements apply β the E/M must be separate from any procedure performed.
Hospital Considerations
For facility billing, modifier 25 is used on hospital outpatient E/M codes (G0463) when a procedure is performed on the same day. Hospital facility E/M has specific billing rules β check the OPPS rules. For ED visits with procedures, modifier 25 is commonly used.
Professional Billing Considerations
For professional billing, modifier 25 is critical for same-day E/M and procedure billing. The E/M must be documented as separate. Check payer prepayment edit programs that target modifier 25 claims.
Never Add Without Documentation
Never add modifier 25 to a claim without supporting documentation. Always verify payer-specific rules and consult the official CPT guidelines, NCCI Policy Manual, and payer policies. When in doubt, consult a certified coder or billing specialist.
